United Jiu Jitsu Waiver of Liability/Informed Consent

In consideration of being allowed to participate in the activities and programs of United Jiu Jitsu and to use its facilities, equipment and machinery in addition to the payment of any fee or charge, I do hereby waive, release and forever discharge United Jiu Jitsu and its officers, agents, employees, representatives, executors, and all others from any and all responsibilities or liability from injuries or damages resulting from my participation in any activities, or my use of equipment or machinery in the above mentioned activities. I do also hereby release all of those mentioned, and any others acting upon their behalf, from any responsibility or liability for any injury or damage to myself, including those caused by the negligent act or omission of any of those mentioned or others acting on their behalf, or in any way arising out of or connected with my participation in any activities of United Jiu Jitsu or the use of any equipment at United Jiu Jitsu. 

I understand and am aware that martial arts, self-defense, strength, flexibility, and aerobic exercise, including the use of equipment, is a potentially hazardous activity. I also understand that fitness activities involve a risk of injury or even death, and that I am voluntarily participating in these activities and using equipment and machinery with knowledge of the dangers involved. I hereby agree to expressly assume and accept any and all risks of injury or death.

I do hereby further declare myself to be physically sound and suffering from no condition, impairment, disease, infirmity or other illness that would prevent my participation or use of equipment or machinery except as hereinafter stated. I do hereby acknowledge that I have been informed of the need for a physician’s approval for my participation in martial arts/self-defense and exercise/fitness activity or in the use of exercise equipment and machinery. I also acknowledge that it has been recommended that I have a yearly or more frequent physical examination and consultation with my physician as to physical activity, exercise and the use of exercise and training equipment so that I have his/her recommendations concerning these fitness activities and equipment use. I acknowledge that I have either had a physical examination and have been given my physician’s permission to participate, or that I have decided to participate in activity and use of equipment and machinery without the approval of my physician and do hereby assume all responsibility for my participation and activities, and utilization of equipment and machinery in my activities.

I understand that at this event or related activities, I may be photographed. I agree to allow my photo, video, or film likeness to be used for any legitimate purpose United Jiu Jitsu producers, sponsors, organizers, and assigns. I understand that this event will happen over the series of events. I consent to having my photo taken in addition to being captured on video to promote the United Jiu Jitsu brand.

The accident waiver and release of liability shall be construed broadly to provide a release and waiver to the maximum extent permissible under applicable law. I CERTIFY THAT I HAVE READ THIS DOCUMENT, AND I FULLY UNDERSTAND ITS CONTENT. I AM AWARE THAT THIS IS A RELEASE OF LIABILITY AND A CONTRACT AND I SIGN IT OF MY OWN FREE WILL.

COVID-19 Liability Waiver

Assumption of the Risk and Waiver of Liability Relating to Coronavirus/COVID-19.

The novel coronavirus, COVID-19, has been declared a worldwide pandemic by the World Health Organization. COVID-19 is extremely contagious and is believed to spread mainly from person-to-person contact. As a result, federal, state, and local governments and federal and state health agencies recommend social distancing and have, in many locations, prohibited the congregation of groups of people.

United Jiu Jitsu has put in place preventative measures to reduce the spread of COVID-19; however, United Jiu Jitsu cannot guarantee that you or your child(ren) will not become infected with COVID-19. Further, attending United Jiu Jitsu could increase your risk and your child(ren)’s risk of contracting COVID-19. I understand that I must follow United Jiu Jitsu’s guidelines to limit the potential of infection with COVID-19.

By signing this agreement, I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that you may be exposed to or infected by COVID-19 by attending United Jiu Jitsu and that such exposure or infection may result in personal injury, illness, permanent disability, and death. I understand that the risk of becoming exposed to or infected by COVID-19 at United Jiu Jitsu may result from the actions, omissions, or negligence of myself and others, including, but not limited to, United Jiu Jitsu employees, volunteers, and program participants and their families.

I voluntarily agree to assume all of the foregoing risks and accept sole responsibility for any injury to my myself (including, but not limited to, personal injury, disability, and death), illness, damage, loss, claim, liability, or expense, of any kind, that I may experience or incur in connection with my attendance at United Jiu Jitsu. On my behalf, and on behalf of my children, I hereby release, covenant not to sue, discharge, and hold harmless United Jiu Jitsu, its employees, agents, and representatives, of and from the Claims, including all liabilities, claims, actions, damages, costs or expenses of any kind arising out of or relating thereto. I understand and agree that this release includes any Claims based on the actions, omissions, or negligence of United Jiu Jitsu, its employees, agents, and representatives, whether a COVID-19 infection occurs before, during, or after participation in any United Jiu Jitsu program.
